Community Care Upstate – PRISMA Health
What is the Community Care Program?
The Community Care Program consists of a group of volunteers who work together to provide practical, emotional and spiritual support to patients in the community. They assist patients who need basic assistance in their homes. The scope of this program is to meet
non-medical needs
. A team's activities may include transportation to a doctor's appointment or grocery store, household or yard chores, errands, meals, social outings, visits, phone calls, prayers, relief for primary caregivers, etc.
Population Served:
Vulnerable patients discharged from Greenville Memorial Hospital identified as high-risk for re-admission and/or two or more of the following criteria:
The patient is chronically ill.
The patient is uninsured or underinsured.
The patient is not homebound but requires additional assistance.
Volunteer Teams:
A team can consist of any number of members – typically 2 to 20. Each team has a Team Captain who will be the point of contact for team members and coordinate the team's schedule. The Program Staff will provide team members with training and ongoing support. Volunteers will choose how many hours they serve each month and designate the skills they have to offer for the discharged patient.

Foster & Adopt Resource Center
Serves and supports over 35 foster, adoptive and kinship care families each month. Opportunities to serve include:
Help receive, sort and organize donations of needed items.
Help distribute items to families who visit.
The Resource Center is located at 2701 Wade Hampton Blvd. It is open to the public on Tuesdays 9–10 am, Thursdays 6:30–7:30 pm, and the second Saturday of each month 9–10 am. However, there are opportunities to serve at other times.

Generations Group
Generations works with adolescent and pre-adolescent males who are in primary need of specialized services for abusive behaviors. A therapeutic environment promotes and rewards positive change.

A baseball organization for special needs children in the Upstate.
Allows developmentally and physically challenged children to play baseball in a non-judgemental, fun and social environment.

The Blood Connection Blood Drive
Supports healthcare partners with adequate, safe, cost-effective blood supplies and services. Brookwood is an established emergency donation site.
